Credit Request Form

This form is for those of you who recieved a credit with your previous order because of an unavailabililty. You should have the amount of credit on the slip of paper included in the package.

YOU MAY :

• Request a Paypal refund for the credit amount.
• Make another order, and have me add extra item(s) in your parcel to make up for the credit amount.

NOTE: If you plan to use your credit toward another toy, do not add it to your shopping cart and go through check out. Simply make a new order and then fill out this form. I will add the item you specify to the new order. You may only request other toys be sent to make up for the credit if you plan to, or have made another order!
